We are proud to be one of only 11 hospitals selected by the American College of Cardiology to start a program that will help keep heart failure and heart attack patients well and out of the hospital. This week, we kicked-off that new program.
The Patient Navigator Program will provide cardiac patients with personalized support after discharge and improve coordination of care within the hospital. The goal is to help patients better understand their disease and better manage their health.
