Today, the Centers for Medicare & Medicaid Services (CMS) announced that WakeMed Key Community Care (WKCC) has been approved as one of 123 new Medicare Shared Savings Program Accountable Care Organizations (ACOs).
WakeMed Key Community Care – a collaboration between WakeMed Health & Hospitals, WakeMed Physician Practices and Key Physicians – brings together a significant network of more than 220 independent primary care physicians with a leading independent health system with another 250 primary care and specialty providers.
“WKCC will facilitate patient access to a coordinated health care team focused on delivering efficient, quality care at a reasonable cost,” commented John Rubino, MD, president of Key Physicians. “Streamlining communication with the patient and between health care providers will reduce duplication of services, limit unnecessary tests, ease transitions from hospital to home and help patients maintain good health.”
